Job Description
  • POSITION SUMMARY
    • The Oncology Social Worker facilitates effective assessments of CancerCare patients and their families . The oncology social worker works with many different entities to include but not limited to physicians, nurses, therapists, dietitians, pharmacists, state and local social service agencies, Trinity volunteers, and community organizations to address the psychological, social and physical needs of the patients through assessment, coordination and monitoring. The social worker facilitates collaboration with all appropriate team members to address CancerCare patients barriers and resource needs. Under the guidance of the Oncology Navigation Supervisor, the oncology social worker is responsible for assisting with and/or coordinating issues related to human resources management, patient assessments, transition of care, discharge planning affecting patient outcomes, volunteer supervision, and patient/family resource programs.
  • MINIMUM QUALIFICATIONS AND REQUIREMENTS
    • Licenses and Certifications Required
      • Currently is a licensed certified social worker, and/or licensed social worker in North Dakota.
      • BLS certification preferred, but not required.
    • Educational Requirements
      • Degree in Social Work, as evidenced by licensure.
    • Experience Requirements
      • Previous experience in oncology preferred.
      • Minimum 1 year experience in health care setting.
      • Hospice and end-of-life care strongly preferred.
